A long-term evaluation of anterior knee pain due to articular cartilage lesions of lateral subliming patellae and idiopathic chondromalacia patellae revealed the effectiveness of a modified lateral release procedure. At five years there was an overall success rate of 72%, but this fell to 63% by 7.5 years, indicating a progressive lesion in some cases, which required a resurfacing operation, but grade 1 and 2 lesions responded and the majority (75%) of these operations were carried out as day cases.
